EPO --> RBC's

TPO (romiplostim --> Platelets

G-CSF (filgrastim)--> Myeloblasts (neutrophil, basophil, eosinophil)

INFy --> granulomas

IL-8 --> neutrophil chemotaKxis

As @yotsubato mentioned, pt has neutropenic fever. That is priority for someone undergoing chemo.

  1. ALWAYS acknowledge the pt's problem, distress, situation, etc. The corollary to this rule is to first make a statement acknowledging an issue without sounding harsh, then asking a non-passive aggressive follow up question to get to the heart of the matter.

  2. NEVER ask the pt to lie

  3. NEVER be a dick. The answer may sound robotic, but should never be mean.

  4. NEVER refer the patient to another resource (in this case, the nurse, but could also be risk management, therapist, etc.)

  5. COMMUNICATE. Talk to other clinicians/experts, etc. to resolve issues.

Disregard for the rights of others with lack of remorse. Involves criminality, impulsivity, hostility, and manipulation.

Classified as conduct disorder if <18.

Antisocial personality disorder is a Cluster B personality disorder that requires evidence of conduct disorder. In this case, the armed robbery when the pt was 16.

7-dehydrocholesterol is a form of pre-vitamin D3 synthesized in the skin from UBV. It is converted to Cholecalciferol, which is the inactive form of Vitamin D3 that is actually in first aid.

anjum  RIP Yang gang +

In addition to all the great points already made, FA also says that Azithromycin is first-line and favored in general because it's a one-time treatment. You would use doxy though to treat lymphogranuloma venereum (FA 2019 and 2020, pg 148-149).

Neisseria gonorrhea - ceftriaxone

  • chlamydia coinfection: +azithromycin or doxycycline

Chlamydia - azithromycin (1st line); doxycycline (2nd line, or for LGV)

  • gonorrhea coinfection: +ceftriaxone
anjum  It's probably simpler to just memorize treatment as: chlamydia: azithromycin gonorrhea: ceftriaxone It's correct and won't trip you up on pregnant women questions +

anjum  To clarify: the lack of DHT production caused by finasteride leads to gynecomastia and ED. The other options are synthetic androgens. Totally agree that this question goes against the benevolence vs. maleficence that we've been taught in medical school +

anjum  shortness of breath = Left heart failure +2
cassdawg  The way I approached the question was a little different: I realized it was heart failure and left would cause lung backup and right the venous backup. When addressing the +3
cassdawg  *the question my thinking was CO = HRxSV; in heart failure we are not pumping as much blood so lower SV (and on top of that his HR is not increased and I think the low blood pressure indicates decompensated heart failure) so CO is definitely lowered. Then CVP would have to be increased because that is how we get the edema (and backup into the venous system increases CVP). +
