Pseudoseizures are diagnosed in EMUs with video EEG. You match the behavior with the EEG to see if there are epileptiform discharges that correspond to the episodes. While this is clearly absurd for an epileptic seizure (impossible movements, insane duration, and no postictal period), there are plenty of cases of bizarre seizure patterns that are VEEG proven.

etherbunny  I guess gone are the days when one might support the airway during a seizure with an oropharygeal airway to see if the gag reflex is intact... +
kalam  wtaf? She's not having a seizure anymore. Why the fuck would VEEG be useful in a patient sitting there looking at you? +1

len49  @kalam, this will be the case for patients with real bonafide seizures or pseudo ones. They might seize once and then you place the EEG on for 24-72 hours and not capture anything, but it is still worth the effort because they may have another while hospitalized. +1

What a dumb question, there's literally a UWorld question about how a girl with a hx of seizures faked one and how you need to do neuro testing if they have no post-ictal confusion. I'm sorry, why are we even entertaining this if she has NO post-ictal confusion?
