  • In children with new development of joint pain, two major diagnoses to consider are septic arthritis and transient synovitis
  • Our patientโ€™s presentation more consistent with septic arthritis due to fever and refusal to bear weight, and therefore patient requires immediate drainage of joint space even before administering antibiotics because of risk for persistent damage to joint
  • Transient synovitis: Afebrile, history of previous viral URI, normal ESR, normal WBC, normal joint aspiration, self-limited condition

Don't pick goofy answers like me.

Do arthrocentesis before ABX, not after.

Suspicion for septic arthritis warrants both diagnostic and therapeutic joint aspiration followed by empiric systemic antibiotics (covering S. aureus in this healthy child).
