This is staphylococcal toxis shock syndrome, commonly associated with tampon use or nasal packing (as in this case). TSST1 (toxic shock syndrome toxin) is a superantigen that nonspecifically activates T-lymphocytes by binding to MHC II and the TCR. (FA2020 p135)

• MOA : Superantigen activation

Lipase degradation of lipids- C. perfringens

Toxin-B mediated cleavage of desmoglein-1 = Exotoxin of S aureus that causes SSSS

Alpha toxin mediated depolarization of plasma membrane - C difficile

Leukocidin-mediated lysis of phagocytic cells- PV toxin of S aureus
